Hello! I had induced lactation for an adoption that ended up failing last year... I pumped for a while afterwards, but then decided to put adoption off while I tended to my mother who was sick.

Anyway, I got a call from the same expectant mother... I know I know... be cautious... she has never asked for any $$, and has become a genuine friend of mine, so I trust her enough to try again.

I don't know how to go about inducing lactation again, should I go ahead with the accelerated method, even though the induction date is scheduled for 3 weeks, but could be sooner? Should skip the birth control and just do dom and pumping? If I do the accelerated method, what do I do when baby is born? I can't get out until Tuesday to buy any supplies, I live close to the border wtih Canada and have a doctor who will co-sign my prescriptions so I don't have to wait on the mail. I just need a new pump (gave mine away).

Last year I was very successful, significant breast changes after about 2 weeks on the pill, and was pumping enough for a newborn in about 2 weeks after I started pumping.

Next question... I couldn't bring myself to throw away or even give away that milk I worked so hard for... I actually have a freezer full of breast milk... probably close to 1000 ounces. It has been in there since last summer. Some of it coming up on a year. Do I need to do anything to it? Is it expired?

Thank you so much in advance for the help!! It has been a whirlwind few days. I have to get my homestudy updated... at first we thought we would give up on the idea of breastfeeding this child, with the short notice. I just can't though... I breastfed my others and I don't want to go to formula. I am sure I can do this... just not sure the right approach to take to ensure a good supply.

Thank again!!

Wow! Congratulations! I don't have any advice since I just started this whole process but good luck - sounds like you won't have a problem...

If you have kept your milk in a deep freeze then its fine. If I were you I would do the accelerated protocol. That way you can kick start your breasts again. Then you can use the lact-aid at the breast with your frozen milk.

I agree with Nona do the accelerated protocol and use your frozen milk in a lact-aid.
If you end up with expired milk (more than a year old) just pastuerize it and it puts you back to day one and you can refreeze it. If you need the pastuerization instructions let me know.

OH and Congratulations. smile

OK... just to make sure I understand...

Do the birth control and the dom until the baby is born, then as soon as the baby is born stop the b/c add herbs in and bf with the lactaid? Should I pump in between feedings or just after baby eats for a few minutes ... and... how do I know when I can start to wean off the lactaid? I am very lucky... my husband gets several weeks of family leave when the baby is born, so I have him to help.

Do you still recommend the Yasmin? Just want to be sure I get the right b/c. I have to make a same-day apt at my dr's tomorrow to get my physical signed for the homestudy update too... so I will do all at once.

I have to double check the dates on my milk, I believe that at least half of it will be less than 12 months old... how do I pasturize the milk that is older than a year? Is it as good as fresh?

Thanks so much!!! smile smile smile smile Sorry for all the questions... with a freezer full of milk you'd think I'd be more confident lol

Dear Mommy 2 three,

What most mothers in your situation do is follow the accelerated protocol for a full 30 days without pumping. The medications recommended by Dr. Jack Newman and the doctors at our breastfeeding clinic are Ortho 1/35 (once daily, only active pills, no sugar pills, no periods) and domperidone (10 mg 4 times per day for the first week and then increase to 20 mg 4 times per day).

Once you have completed at least 30 days on the combination of Ortho 1/35 (once a day, note these days we prefer Ortho 1/35 to Yasmin) and domperidone (20 mg 4 times a day) and have experienced significant breast changes you can stop the Ortho 1/35, maintain the domperidone and begin pumping with a double electric breast pump. A hand pump is just not up to the job. The double electric pump fools your body into thinking you have a really hungry set of twins at the breast and is the fastest way to increase your milk supply. The pumps most moms seem to be using are the Medela Pump n Style (buy) and the Medela Lactina Select (rent). A new pump has become available recently that many mothers have started using with excellent results. It's the PJ junior by Limerick. It has flexible breast shields that compress the breast while suction is applied, acting very much like a baby at the breast. It's available through [email protected] but I hear the price is about to go up so you may need to hurry.

You?ll need to pump every 3 hours and once during the night. Once you?ve started pumping you can add the herbs Blessed Thistle herb (approx 390 mg per capsule) and Fenugreek seed (approx 610 mg per capsule). Most mothers take 3 capsules of each 3 times a day with their meals and their domperidone 1/2 hour before meals for best absorption.

They also try to eat oatmeal for breakfast at least 3 times a week and drink 6 glasses of water a day but it's important that you don?t make yourself sick.

You can expect clear drops, which become more opaque. Then spray, then stream. It may take a few days, a week, or two, or more for you milk supply to come in. Everyone is different.

If your baby arrives before you have completed the acelerated protocol you simply maintain your medications, and put your baby to the breast with the help of a supplemental feeding tube device such as a Lact-aid www.lact-aid.com or Medela SNS filled with either donor breastmilk or formula. Once the 30 days are complete, you would stop the Ortho 1/35, maintain the domperidone, add the herbs and continue to breastfeed but try to pump for about 10 minutes after most feeds to build your milk supply.

Please discuss all of this with your doctor so that you can be monitored and educate at the same time.

Keep us posted on your progress. Good luck!

Best, Lenore

I know that many women were using the Yasmin only to have to switch to the Ortho 1/35 to get better results. Maybe they figured it was better to just start everyone out there. smile

I went the other way and had to switch from the ortho 1/35 to the yasmin for changes.

I think I will just go with the Yasmin, because I was very successful last time around...

Yes, the milk is in my deep freeze.

Thank you everyone for your help again! I will update with my milk progress, and hopefully baby pictures soon!! smile smile smile
